Some dealerships have submitted forms that appear to be modified and altered. This includes the checkbox or box around text being removed. Examples are shown below.

These forms are not acceptable and any DR forms provided by the Colorado DMV must be filled out and submitted unaltered. The two forms we have seen this occur on the most are the DR2842 and the DR2698.
